The short answer

Ordinary U.S. licence holders have the MIT one-year/IDP-or-official-translation boundary for driving in Italy. The MIT preserves a diplomatic exception, but for most Americans, there is no automatic entitlement to simply exchange a U.S. state licence for an Italian one.

The One-Year Boundary

If you become a resident of Italy, you may generally drive on your valid U.S. licence for a maximum of one year from the date you establish residence. During this time, your U.S. licence must be accompanied by an International Driving Permit (IDP) or an official sworn translation in Italian.

After one year of residence, the U.S. licence is no longer recognized for driving in Italy. An IDP does not extend this one-year limit.

No Ordinary Exchange

Unlike some countries, the United States does not have a comprehensive reciprocal licence exchange agreement with Italy for ordinary drivers. The MIT only preserves an exception for diplomatic personnel and their families.

This means that to continue driving legally after the first year of residence, an American must generally go through the full Italian licensing examination process since conversion is unavailable.

Do not confuse diplomatic or special military exceptions with ordinary civilian rules. If driving is essential to your life in Italy, factor the Italian driving school (autoscuola) and exam process into your relocation timeline.
